I didn’t catch the name / I didn’t catch your name.

お名前を聞き取らなかったのですが。

「名前は何ですか？」でも使えます。

A: Sorry, but I didn’t catch the name.

B: It’s Aki. And yours?

A: すみまえん、お名前が聞き取れなかったのですが。

B: アキです。あなたは？

A: By the way, I didn’t catch your name.

B: I’m Taka Suzuki. B: It’s nice to meet you. I’m Kei Tanaka.
