Shape は「形」。

本来の「形」から「out of shape (外れる)」となり、意味は「形から外れる」という意味となります。

そこから「体が怠けている」「運動不足」「体調が悪い」という意味で使われます。

I am getting out of shape lately. I have to to go the gym. 最近、

怠けているな。ジムに行かなくては。

I’m out of shape. I am going to a clinic.

体調が悪い。病院に行ってくるよ。
